Navigating through the challenges of aging can be overwhelming. Heartland’s Outreach Department works to connect seniors in Story County with community resources that they may be unaware of.

Through home visits and telephone contacts, our Outreach Specialists provide a vital link between older adults and necessary community resources, enabling seniors to remain in their homes for as long as possible.  We are always willing to listen and lend a helping hand.

The Outreach Department is an excellent resource for seniors or their families who have questions or concerns regarding older adult issues. We meet with people, listen to individual needs, and work to develop relationships that will bring a higher quality of life to those we serve. “One lady in our community struggles with depression and diabetes, and has many other health issues. She has been very sick off and on. As we worked to set her up with home delivered meals, we found out that she had no income. We were able to assist her with getting signed up for food stamps and social security benefits. She now receives a monthly social security check and is able to purchase food regularly. These three changes have made a big impact in her overall health and well-being.”
